Appointments 

When booking an appointment via any of the routes below you will be directed to the most appropriate member of our multidisciplinary team to deal with your health problem. We have a range of specialist healthcare practitioners at the surgery including; physiotherapists, social prescribers, physician associates, paramedics, nurses, healthcare assistants and pharmacists who work closely with the doctors. - View the video to find out how to complete a triage and to see what happens when we receive it!  Please note these services are for Spring Hill Practice patients only.


Book an Appointment Online 

To make an appointment for a new or ongoing medical issue or to receive administrative help with a GP letter, sick note or test result, please fill out an online form via AccuRx. Same day appointments can be offered if needed. The online triage form is available from 7am to 5pm Monday to Friday. - Please note these services are for Spring Hill Practice patients only. 

Call the Practice 

If you are unable to complete the triage form online you can call the practice on 0208 806 6993 between 8AM and 5PM. Select option 2 for a patient navigator and we can help you complete the triage form for an appointment with a doctor or pharmacist. 

Please keep in mind that you can still call at any time between 8AM-7:30PM for any other queries and appointments with our nursing team.

Book an Appointment via NHS app 

You can book an appointment with our nurses and healthcare assistants via the  NHS app.

Administrative requests 

Please fill out an Online form via accuRx to request a fit note, enquire about test results or to enquire about administrative help with a letter or form. An appointment will be arranged for you within 48 hours if needed.  You can still call the practice if you don't have access to online services. The online service is only available Mon-Fri between 7AM and 5PM

Urgent Appointments

All online triage forms for medical issues will receive a triage outcome on the same day. Please include the information about why your issue is urgent on the triage form. You can also access urgent same day appointments by phoning reception to complete the form over the phone. We have a duty doctor available from 8am to 6.30pm daily. - Please note these services are for Spring Hill Practice patients only.  

Life Threatening - Go to A&E

If you or someone you know has a life-threatening condition, such as loss of consciousness, a sudden confused state, chest pain, breathing difficulties etc, you should call 999


In an EMERGENCY call 999.
Emergencies are situations that cannot be managed at home and maybe life threatening

Continuity of Care

We encourage our patients to stay with one doctor for continuity and all registered patients are allocated a ‘usual GP’. We recognise that continuity of care improves patient satisfaction and leads to better health outcomes. Please fill out an online admin form or contact reception if you would like to know who your usual GP is.  

Appointment SMS Reminders

We will send you an automated text message 24 hours in advance of your appointment to remind you. If you no longer wish to attend the appointment, please reply to the text message with the words CANCEL.  This will remove the booked appointment and allow another patient to use it.

How to Cancel an Appointment

If you are unable to keep your appointment, please give us as much notice as possible so that your appointment can be offered to someone else.

If you are cancelling please telephone us on 020 8806 6993 as soon as possible, and at least 30 minutes before your appointment time. This will enable us to offer your appointment to someone else.

Alternatively, you can cancel your appointment online using Patient Access or The NHS App if you are signed up to the service.

Please remember that there may be a shortage of appointments and when you fail to attend you have prevented someone else from being seen in your place. On average about 230 appointments are missed each month

Cancellations notified less than 30 minutes before the appointment time will be recorded as failure to attend (DNA).

Repeated failure to attend booked appointments is a significant waste of NHS resources

Primary Care GP Extended Access Centres

There are now Extra GP appointments in the evenings and at weekends available to City and Hackney patients.

Urgent and routine appointments with a GP, either same day or in advance are available at any one of our five primary care GP practices, making it much easier to get the care you need at a location that is convenient for you.


Who can access the service?

Patients registered with a City and Hackney GP practice can book an appointment at the service through their GP practice. These appointments are not suitable for all types of problems. (NB This is not a walk-in service) 

What can I expect at a consultation with a GP?

One of the added benefits of the primary care extended access service is that the GP will have access to view your medical record during your visit to the primary care access centre. Having access to view your medical record means that the clinician will have a greater understanding of your medical history and health needs and can then choose the treatment that is right for you. The clinician will ask you for your consent before accessing your medical record.

Who will have access to my medical record?

Once you have consented, only authorised staff within the service will have access to view your medical record. Clinicians can only review your medical record if you have given your expressed consent. If you do not consent then the GP will not be able to continue the consultation. Your medical information will be treated with the strictest of confidentiality.  All City and Hackney GP practices have agreed to sign up to a data sharing agreement which sets out what information will be shared, how and when.

What if I don’t want my medical records to be shared?

If you don’t want your medical records to be viewed then you will need to tell the receptionist or GP at your own practice when they are booking the appointment. They will then offer you an appointment at your own GP practice.

Your views and feedback about the primary care extended access service

At the end of the consultation with the GP you will be asked to complete a patient experience survey. Your views and feedback are important and will help us to make improvements.

Tests and referrals

The GP’s in these centres will send any requests for tests and referrals back to your own GP practice to action – in a small number of cases your own GP practice might recommend a different management plan to the extended access GP.
